Base pay range

$58,000.00/yr - $63,000.00/yr

Medical Social Worker – Hospice

Medi Home Hospice, a proud member of the Medical Services of America, Inc. family, currently seeks an experienced full-time Medical Social Worker to join our Hospice team to cover Pittsburgh and the surrounding areas.

Responsibilities

• Provide social work services in accordance with the physician’s plan of treatment.
• Work with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.
• Provide assessment for normal and complicated grief and provide bereavement counseling as appropriate.
• Serve as a liaison with other community resources.

Requirements

• Master’s or Bachelor’s level social work degree from a school of social work accredited or approved by the Council of Social Work Education.
• Minimum of one-year Hospice experience preferred.
• Valid Driver’s License.
