Job summary

A leading healthcare provider is seeking a Full-Time Medical Social Worker to join the Hospice team to cover Rock Hill and surrounding areas. This role includes providing social work services as per the physician’s treatment plan and working with families. Candidates should have a Master's or Bachelor's degree in social work, with hospice experience preferred. Qualifications

Base pay range

$50,000.00/yr - $65,000.00/yr

Responsibilities Include:

· Provides social work services in accordance with the physician’s plan of treatment.

· Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.

· Provides assessment for normal and complicated grief and provides bereavement counseling as appropriate.

· Serves as a liaison with other community resources.

Job Requirements

· Master's or Bachelor's level social work degree from a school of social work accredited or approved by the Council of Social Work Education.

· Minimum of one‑year Hospice experience preferred.

· Valid Driver’s License.
