Medi Home Health & Hospice, part of the Medical Services of America family, is seeking a Hospice Nurse to serve patients in the Roanoke, VA area. The role requires an RN license in VA and a valid driver’s license to travel to patient homes as needed.

The Hospice Nurse will regularly reevaluate nursing needs, develop and revise plans of care, and provide compassionate skilled nursing care aligned with physician orders and agency policy.

Qualifications

• Completion of an accredited nursing program.
• Must be currently licensed as an RN in VA.
• Hospice experience preferred.
• Valid Driver's License required.

Responsabilités

• Provides skilled nursing care to clients as directed by the RN and physician’s plan of care in accordance with policies.
• Prepares clinical progress notes and follows client care plans.
• Initiates diagnostic, preventive and rehabilitative nursing procedures as appropriate to the client’s care and safety.
• Observes, records and reports the client’s reaction to treatment and any change in condition.
• Teaches, supervises and counsels the client and family regarding hospice nursing procedures.

Medi Home Health & Hospice, a proud member of the Medical Services of America family is currently seeking an experienced Hospice Nurse to service patients in the Roanoke (Salem, Vinton, Franklin County, Moneta, Boones Mill, Botetourt, Buchanan, Floyd, Bedford), VA area. Regularly reevaluates the nursing needs of the client; initiates, develops, implements and makes necessary revisions to the client’s plan of care.

• Provides skilled nursing care to clients as directed by the registered nurse and physician’s plan of care in accordance with agency policies
• Prepares clinical progress notes and follows client care plans as developed by the registered nurse and according to agency policy.
• Initiates diagnostic, preventive and rehabilitative nursing procedures as appropriate to the client’s care and safety.
• Observes, records and reports the client’s reaction to treatment and any change in the client’s condition.
• Teaches, supervises and counsels the client and family regarding hospice nursing procedures and other care needs as appropriate to the client’s condition.

Job Requirements

• Completion of a nursing program from an accredited school or university required.
• Must be currently licensed as a RN in the state of VA.
• Hospice experience preferred.
• Valid Driver's License required.
