Oak Street Health in Dallas is seeking a Medical Social Worker to join our care teams and support the psychosocial needs of high‑risk patients and their caregivers. In collaboration with the interdisciplinary team, you will provide assessments, advocacy, and comprehensive care coordination to improve health outcomes and reduce hospitalizations.

The role requires an MSW from a CSWE‑accredited program, state licensure, fluent communication in Spanish or other community languages, and proficient

Qualifications

• MSW degree from CSWE-accredited program.
• Licensed (or minimally licensed) in the state where OSH Center is located.
• Fluency in specified languages used in the communities served.
• Proficient computer skills with EMR systems.
• US work authorization.

Responsabilités

• Proactively assess high-need patients with telephonic and in-person visits.
• Coordinate care with patients, families, and care teams to meet goals.
• Referrals to community resources to address health and psychosocial needs.
• Navigate insurance benefits with the team to maximize plan usage.
• Provide longitudinal and episodic care coordination to improve health.
• Educate patients on care options and goals of care throughout the journey.
• Provide psychoeducation on life stage and disease-specific issues.
• Collaborate with care teams to develop creative solutions.
• Document clearly to ensure visibility across the care team.
• Mentor Community Health Workers and provide clinical guidance.
• Respond to crises and prioritize tasks in urgent situations.
• Other duties as assigned.
