Job summary

Oak Street Health in Dallas, TX seeks a Medical Social Worker to support the psychosocial needs of patients and collaborate with interdisciplinary care teams to coordinate care for our highest-risk patients and their caregivers.

You will provide assessments, advocacy, and comprehensive care coordination to improve health status and reduce hospitalizations, while navigating resources and insurance benefits. This full-time role reports to the Practice Manager.

Qualifications

• MSW degree from an accredited program.
• Licensed minimally in the state where OSH Center is located.
• Fluency in Spanish, Polish, Russian, or other languages spoken by communities served.
• Proficient computer skills (Microsoft, GSuite, EMR).
• US work authorization.

Responsibilities

• Provide comprehensive assessments and telephonic/in-person check-ins for high-need patients.
• Partner with patients and support persons to set goals and execute care plans.
• Refer to community resources to meet health and psychosocial needs.
• Navigate insurance benefits with PRM to maximize plan benefits.
• Coordinate care to empower patients to manage health conditions.
• Educate on supportive care options and goals of care counseling.
• Provide psychoeducation to patients and families on relevant topics.
• Collaborate with care teams to develop creative solutions for complex psychosocial issues.
• Document interventions concisely and timely.
• Guide Community Health Workers with clinical consultation.
• Respond to crises with prioritization and multitasking.
• Other duties as assigned.

Education

Master of Social Work (MSW) degree, CSWE-accredited
