Job summary

Responsibilities

• Conduct biopsychosocial assessments of patient/family situations.
• Document assessments and interventions in the medical record to ensure continuity of care; develop care plans and evaluate outcomes.
• Assist the health care team in understanding cultural, economic, emotional and family factors.
• Provide supportive counseling to patients/families adjusting to disabilities, illness, loss, and bereavement.
• Assist patients/families in utilizing community resources based on needs.
• Maintain service excellence through positive relationships with patients, physicians, co-workers and referral sources.
