Qualifications

• Master’s Degree in Social Work and licensure in Minnesota (LGSW) required.
• Experience in healthcare setting preferred.
• Experience using electronic health records preferred.
• Proficient in Microsoft Office products including Word and Outlook.
• Able to communicate effectively in written and verbal form.
• Ability to manage multiple tasks and stay calm in emergencies.
• Experience handling volatile and potentially explosive patients after training.

Responsibilities

• Accountable for knowledge of Social Work practices and principles.
• Conduct psychosocial and suicide risk assessments, discharge planning, and lead groups.
• Develop treatment and discharge plans; conduct discharge meetings.
• Communicate with patients and families to involve them in treatment decisions.
• Coordinate care with outpatient providers on admission and discharge.
• Lead weekly treatment team meetings and maintain master treatment plans.
