Qualifications

• Bachelor’s degree in Social Work or related field from an accredited program.
• Healthcare and/or mental health hospital experience preferred; BLS certification within six weeks of employment.
• Must possess a license in good standing in state(s) of practice: Wisconsin State Social Work license within two years of hire.

Responsabilités

• Provides supportive services and counseling on healthcare and home care programs and services.
• Serves as a member of the interdisciplinary team coordinating social, emotional and economical concerns of patients and families.
• Responds to suspected abuse, neglect or violence per NASW Code of Ethics and state laws.
• Develops appropriate plan of care by obtaining resources from social, health and human services agencies.
• Provides referrals and education regarding programs and services available.
• Demonstrates professionalism and active participation in care conferences and rounding.
