NorthBay HealthCare Group seeks a Medical Social Worker in Fairfield, CA to perform psychosocial assessments, case management and crisis intervention across inpatient and outpatient settings. You will collaborate with physicians, pharmacists and care teams to support treatment engagement and transition planning.

The role requires an MSW, CA ASW, and strong communication and clinical decision-making skills. Experience in addiction medicine and EMR documentation is preferred.

Formación

• MSW degree from a CSWE-accredited program.
• California ASW required unless grandfathered with 40+ years of experience; LCSW preferred.
• Minimum two years' experience in acute health care or community health settings.

Responsabilidades

• Provide psychosocial assessment, case management, crisis intervention and care coordination.
• Collaborate with interdisciplinary teams to support treatment engagement and recovery.
• Document in EMR and maintain patient records per policy.
• Develop plan of care addressing social, emotional and financial needs.
• Support transitions across inpatient and outpatient settings.
