Madrona Hospice Care Inc. is seeking a Medical Social Worker to lead the social services team in home-based patient care on a per diem basis.

The role includes clinical supervision, policy oversight, and coordination with other clinical providers to maintain compliant, high-quality service levels. The successful candidate holds an MSW, active CA LCSW license, and at least 3 years of management experience in home care, with strong knowledge of ACHC standards and healthcare regulations.

Qualifications

• Master of Social Work (MSW) from an accredited program.
• Active California license as an LCSW.
• At least 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations and ACHC standards.
• Excellent communication skills and the ability to lead a team.

Responsibilities

• Lead daily operations of the social services team, including policies, procedures, and care coordination.
• Recruit, onboard, and mentor social workers—providing clinical supervision and supporting professional growth.
• Oversee assessment, planning, and evaluation of social services care and ensure documentation meets standards.
• Partner with the Executive Director on department budget and resource planning.
• Build knowledge of community resources and connect patients and referrals to support.
• Plan in-service training and continuing education to keep skills sharp.
• Contribute to quality improvement initiatives and identify new ways to serve patients.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $100 - $120 per visit.
