Job summary

Madrona Hospice Care Inc. seeks a Medical Social Worker to shape patient home-based care, mentor social workers, and uphold high-quality service standards across the organization.

The role is per diem with a rate of $100 - $120 per visit, offering opportunities for full-time conversion based on performance. Responsibilities include leading the social services team, supervising clinicians, ensuring compliant documentation, and collaborating on budgeting and resource planning.

Qualifications

• MSW from an accredited program.
• Active California LCSW license required.
• At least 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations, ACHC standards, and business operations.
• Excellent communication and relationship-building with patients, families, staff, and community.

Responsibilities

• Lead daily operations of the social services team, including policies and care coordination with clinical providers.
• Recruit, onboard, and mentor social workers with supervision and professional development.
• Oversee assessment, planning, and evaluation of social services care and ensure documentation meets standards.
• Partner with the Executive Director on budget and resource planning.
• Build knowledge of community resources and connect patients/referral sources to support.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards
• Partner with the Executive Director on the annual department budget and resource planning
• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services
• A Master of Social Work (MSW) from an accredited program
• An active California license as an LCSW
• At least 3 years of management experience, preferably in home care
• Strong knowledge of healthcare regulations, ACHC standards, and business operations
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community
