Job summary

Madrona Hospice Care Inc. is seeking a Medical Social Worker to lead home-based social services, mentor staff, and set standards for compassionate service.

This per diem role requires an MSW, an active California license as an LCSW, and at least three years of management experience in home care. You’ll collaborate with clinical teams, manage care coordination, and contribute to quality improvement.

Per visit pay ranges from $100 to $120, with potential to convert to full-time based on performance.

Qualifications

• MSW from an accredited program.
• Active California license as an LCSW.
• At least 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations, ACHC standards, and business operations.
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community.
• A self-directed, organized, and flexible approach—you take initiative and work well with others.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards
• Partner with the Executive Director on the annual department budget and resource planning
• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services
