Job summary

Alternate Solutions Health Network is seeking a Medical Social Worker (MSW) in our Beaumont Home Health and Hospice service area. You will work closely with interdisciplinary teams to ensure patients receive holistic care in their preferred environment.

The role emphasizes planning care, supporting families, and navigating community resources, with a focus on compassionate communication, attention to detail, and adherence to Medicare guidelines.

Qualifications

• Master’s degree of Social Work is required.
• Licensed MSW in the state of employment is required.
• Minimum one year experience as an MSW in acute care setting.
• Home care experience is preferred.

Responsibilities

• Plan of Care: participate in developing the Plan of Care and assess patient needs.
• Patient outcomes: connect patients and family to community resources and assist with financial aid applications.
• Family educator/advocate: counsel patient and family on coping and grief.
• Policies: complete clinical documentation per agency protocol and Medicare guidelines.
• Rules and regulations: follow policies and communicate schedule changes.
• Operations: attend in-service trainings and mandatory meetings.

HOW YOU'LL WORK

You'll provide patients with access to community resources and ensure that the patients’ medical, emotional and safety needs are met. You'll provide comprehensive skilled services as ordered by the attending physician. You will educate patients and their family members and ensures the safety of the patient.

MAJOR AREAS OF RESPONSIBILITY

• Plan of Care: Participate in the development of the Plan of Care and Conduct assessments of the patients’ needs and identifies any barriers.
• Patient Outcomes: Connect patients and family to community resources. Assists with patient financial aid applications and long-range planning.
• Family Educator/Advocate: Counsel the patient and family related to coping, changes and grief.
• Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
• Operations: Attend in-service training and mandatory agency meetings.

HARD & SOFT SKILLS

• Compassionate communicator with a positive attitude.
• Patience is a virtue when working with patients, families, physicians, and coworkers.
• Attention to detail is critical, as is being observant and following directions.

REQUIREMENTS

• Medical Social Worker with a current license in the state of employment. Limited licenses are not acceptable.
• Master’s degree of Social Work required.
• Minimum one-year experience as a MSW in an acute care setting.
• Home care experience preferred.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.
