Job summary

Residential Home Health and Hospice is seeking a Medical Social Worker (MSW) to provide skilled nursing care in patients’ homes across Ingham and Eaton Counties, MI. You will work with an interdisciplinary team to address emotional, social, and environmental factors and support families with community resources and planning.

Role requires travel to patient locations, valid MSW licensure, and 1–2 years in healthcare social work.

Qualifications

• Master’s in Social Work from graduate school accredited by the Council of Social Work
• 1–2 years post graduate social work experience in a health care setting
• Must hold, in good standing, a license as a Medical Social Worker issued by the state
• Current driver’s license and ability to drive ~20% of the day to patient locations

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team
• Conduct psychosocial assessments and develop individualized care plans
• Educate and connect patients with community resources and counseling services
• Maintain accurate EMR documentation with timely updates

MSW Qualifications
