Job summary

AdvisaCare Home Health & Hospice is seeking a compassionate Medical Social Worker (MSW) to support patients and families in northern Michigan communities, including Gaylord, Cheboygan, Charlevoix, Traverse City and surrounding areas. The MSW will perform psychosocial assessments, help develop and implement care plans, and provide bereavement support within a dedicated hospice team.

A CSWE-accredited MSW degree is required, with home health or hospice experience preferred and valid driving

Qualifications

• MSW degree from CSWE-accredited program.
• One year of home health care experience preferred.
• Hospice experience preferred.
• Valid Driver's License, car insurance and reliable transportation.

Responsibilities

• Performs psychosocial, emotional, spiritual and bereavement assessments and assists in the development and implementation of goal-directed care plans.
• Documents all patient/family visits, telephone contacts and referrals in the patient record timely and accurately.
• Provides crisis intervention and individual or family counseling when indicated.
• Participates in IDT conferences observing, assessing and bringing information on psychosocial and financial conditions affecting the patient and family.
• Develops and maintains contacts with community agencies to promote referrals and interagency cooperation.
• Provides ongoing counseling related to death and dying to the patient and family as needed.
• Acts as a care advocate for the patient/family unit and adheres to hospice policies.

Requirements

• MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE)
• One year of experience in Home Health Care preferred
• Hospice experience preferred
• Valid Driver's License, car insurance and reliable transportation

Job Duties:

• Performs initial psychosocial, emotional, spiritual and bereavement assessments and assists in the development and implementation of goal-directed care plans
• Provides care under the direction of a physician. Participates in IDT conferences observing, assessing and bringing information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family
• Documents all patient/family visits, telephone contacts and referral actions timely, accurately and concisely in the patient record
• Conducts reassessments of patient/family needs and counseling as required
• Provides crisis intervention and individual or family counseling when indicated
• Plays an active role as a care advocate for the patient/family unit
• Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals
• Provides ongoing counseling related to issues of death and dying to the patient and family as needed
• Participates as a member of the Bereavement Team as assigned
• Adheres to all Hospice policies
