VNA Health at Home, part of a broader health care network, is seeking a Medical Social Worker to support patients at home. You will develop care plans, provide crisis intervention, and connect patients with community resources. Strong collaboration with caregivers and timely documentation are essential.

Minimum qualifications include an MSW, state licensure, and CPR/BLS certification, with at least one year in health care. This position offers hourly compensation in Kentucky.

Qualificações

• Master’s degree in social work from an accredited program.
• Current licensure to practice social work in the applicable state, including Indiana requirement if applicable.
• Minimum of one year of social work experience in a health-care setting.
• CPR/BLS certification current, per American Heart Association or Red Cross; state requirements met.

Responsabilidades

• Establish care plans aligned with physician treatment plans.
• Provide crisis intervention as needed.
• Assist physicians and caregivers in addressing social, emotional and economic factors.
• Educate patients and caregivers on care needs and resources.
• Refer to community services to access public assistance, housing, transportation, etc.
• Collaborate with caregivers to maintain a positive patient–caregiver relationship.
• Complete and submit required documentation accurately and timely.

Job Summary And Responsibilities

At VNA Health at Home, we strive to embody our mission of delivering hope and healing to those we serve. As a Medical Social Worker, you’ll engage in a range of exciting responsibilities, including:

• Establishing a care plan in accordance with the physician’s plan of treatment.
• Providing crisis intervention as required.
• Assisting the physician and other caregivers in understanding the significant social, emotional and economic factors influencing the patient’s ability to improve medically.
• Providing patient care education to patients and caregivers.
• Assisting in making appropriate community referrals to gain access to additional resources as needed (i.e. applications for public assistance, housing problems, additional health care resources, transportation)
• Working with the patient’s caregivers to resolve problems interfering with an optimum caring relationship between the patient and caregiver.
• Completing and submitting all required documentation in an accurate and timely manner.

Job Requirements

Qualifications

• Master’s degree from an accredited school approved by the Council on Social Work Education. In Indiana, must also be certified under IC25-23.6-4-3.
• Currently licensed to practice in the state serviced.
• One year social work experience in a health care setting.
• Current CPR/BLS certification through American Heart Association or American Red Cross and compliance with state requirements

Pay Range

$26.70 - $39.71/hour
