Alternate Solutions Home Health Columbus is hiring a Medical Social Worker (MSW) for a part-time schedule in the Columbus area. You will assess patient needs, develop and monitor the care plan, and coordinate with physicians and staff to ensure comprehensive, patient-centered care.
The MSW will educate families, connect patients to community resources, assist with financial aid applications, and document care per Medicare and agency guidelines. A current MSW license and MSW degree are required.

Territory: Union, Delaware, Madison, Franklin, Licking, Knox, Fayette, Pickaway, Fairfield, and Hocking Counties.
The Medical Social Worker (MSW) provides patients with access to community resources and ensures the patient’s medical, emotional, and safety needs are met. The MSW follows all legal rules and regulations. The Company adopts the social work scope of practice as outlined in the Administrative rules of the state for its licensed MSW personnel. The MSW is responsible for any and all practices and duties within the scope of practice as outlined by the state.
