Kettering Home Care is seeking a Medical Social Worker I (MSW I) to provide access to community resources and ensure patients' medical, emotional, and safety needs are met. The MSW I will follow state rules and regulations and operate within the social work scope of practice as outlined by the state.
The position covers the Greater Dayton and West Central Ohio service area, including many communities. Qualifications include a current MSW license and one year of medical social work experience.

Agency: Kettering Home Care Schedule: Monday - Friday, 8:00AM - 5:00PM Territory: This position covers the Greater Dayton and West Central Ohio service area, including communities throughout the northern and northeastern Dayton region: Maplewood, Pemberton, De Graff, Quincy, Christiansburg, Conover, Casstown, St. Paris, Fletcher, Springfield, Enon, Medway, New Carlisle, Donnelsville, North Hampton, Sidney, Minster, Houston, Fort Loramie, Anna, Port Jefferson, Troy, Westville, Urbana, West Liberty, Tremont City, Lippincott, Bellefontaine, Cable, North Lewisburg, Woodstock, Mechanicsburg, Catawba, and South Vienna.
The Medical Social Worker I (MSW) provides patients with access to community resources and ensures the patient’s medical, emotional, and safety needs are met. The MSW I follows all legal rules and regulations. The Company adopts the social work scope of practice as outlined in the Administrative rules of the state for its licensed MSW personnel. The MSW I is responsible for any and all practices and duties within the scope of practice as outlined by the state.
Frequency: Drives to patient’s primary location per scheduled visit. Daily attendance at assigned visit locations. Documents all aspects of subsequent, discharge, eval/recert/resumption of care visits within 24 hours of visits. Completes and submits all required documentation within specified company requirements.
Fulfills MSW visits in assigned geographic location per patient need within federal and state specifications.
Dexterity & vision to complete documentation on a PDA. Driving, as needed. Current applicable license as a Medical Social Worker. Follows plan of care as permitted within the scope of practice for a Licensed Medical Social Worker (R 338.2901). Other duties as assigned.
Licensed Social Worker with current license in the state of employment. Master of Social Work required.
