The MSW I follows all legal rules and regulations. The Company adopts the social work scope of practice as outlined in the Administrative rules of the state for its licensed MSW personnel. The MSW I is responsible for any and all practices and duties within the scope of practice as outlined by the state.KEY RESPONSIBILITIESConducts assessment of patient needs and identifies any barriersParticipates in the development of the plan of careObserves and monitors patient conditionsInforms the physician, nursing supervisor, and other essential staff of changes in the client’s condition when necessaryEducates team members in understanding significant social and emotional factors related to the health problems of the patient and acts as a consultant to other personnelAssists patient and family to understand, accept, and follow medical recommendations and how to utilize home health care servicesConnects patients and family to community resources and supportAssists with patient financial aid applicationsAssists in long range planningCounsels the patient and family related to coping, changes, and griefListens to patient and family membersCompletes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelinesDocuments all aspects of care, assessments, and patient educationMaintains active social work license in state of employmentCompletes chart audits at agency location as requestedCommunicates with scheduler any changes outside the normalParticipates in all on-call requirements and case conferencesAttends in-service trainings and mandatory agency meetingsMaintains a professional appearance as a representative of the companyUnderstands and practices agency policies and proceduresFrequency: Drives to patient’s primary location per scheduled visit. Daily attendance at assigned visit locations. Documents all aspects of subsequent, discharge, eval/recert/resumption of care visits within 24 hours of visits. Completes and submits all required documentation within specified company requirements.Operational Demands: Fulfills MSW visits in assigned geographic location per patient need within federal and state specifications.Specific Essential functions: Dexterity & vision to complete documentation on a PDA. Driving, as needed.Current applicable license as a Medical Social Worker. Follows plan of care as permitted within the scope of practice for a Licensed Medical Social Worker (R 338.2901).Other duties as assigned.QUALIFICATIONSOne year of Medical Social Work experience required.Ability to create positive impressions and communicate with a variety of people.Maintain effective communication with patients, families, physicians and co-workers.Ability to remain calm, have patience and be accommodating.Compassionate and caring while working with patients.Knowledgeable on social work best practices.Ability to make appropriate judgments.Ability to identify a situation and handle it with the best possible solution.
