Job summary

Amedisys is seeking a medical social worker for a full-time day shift in Massachusetts. You will work to support patients and families, coordinating with the care team to deliver comprehensive social work services at home.

Responsibilities include applying the plan of care, addressing financial and emotional needs, and facilitating safe discharge planning. A strong background in healthcare social work and CPR certification is required.

Qualifications

• Holds a master's or doctorate degree in social work from a program accredited by the Council on Social Work Education.
• Has at least one year of social work experience in a health care setting.
• Meets state and federal requirements for this position.
• Current CPR certification is required.

Responsibilities

• Assists the patient and family with health-related financial, social and emotional concerns and helps with coping skills.
• Provides social work services in accordance with the plan of care.
• Ensures changes in the patient's clinical status are communicated to the health care team and care center staff.
• Educates patient and family on social work interventions and treatment plan.
• Performs care planning, transfer and discharge functions.
• Identifies risks to the patient's health and implements interventions to reduce risks and coordinates care with physicians and team.

Patient Coverage Territory: Middleboro, Wareham, Freetown, Lakeville, Fairhaven, New Bedford, Dartmouth, Westport, Fall River, Dighton, Taunton but orientation will take place at the care center in Bridgewater.

• Provides social work services in accordance with the plan of care.
• Ensures that changes in the patient's clinical status are communicated to other healthcare team members and the care center office staff in a timely manner.
• Educates the patient and family members and other team members on medical social work interventions and treatment plan.
• Performs care planning, transfer and discharge functions.
• Identifies risks or threats to the patient's health in the patient's environment and promptly implements appropriate interventions to reduce risks. Communicates identification of risks/ threats to the patient to the physician and other members of the healthcare team to coordinate care effectively.
• Other duties as assigned.

Qualifications

• Holds a masters or doctorate degree from a school of social work, accredited by the Council on Social Work Education
• Has one year of social work experience in a health care setting.
• Meets state and federal requirements for this position.
• Current CPR certification.
