Job summary

Harvard University Health Services (HUHS) invites a Licensed Independent Clinical Social Worker (LCSW) to join our on-site multidisciplinary care team in Cambridge. You will coordinate patient care with physicians, nurses, and staff, and support patients and families with medical, social, emotional, insurance, and vocational needs.

The role emphasizes care planning, documentation, and collaboration within HUHS standards.

Qualifications

• Master’s degree in social work or related field.
• Minimum of 5 years’ relevant work experience.
• Proficiency in Microsoft Office Suite and intermediate Excel.
• Excellent organizational, analytical, problem solving, and interpersonal skills.
• Experience in a hospital or healthcare setting preferred.
• Knowledge of discharge planning and care coordination preferred.
• Strong communication, collaboration, organization, and clinical assessment skills.

Responsibilities

• Collaborate with physicians, nurses, support staff, and other healthcare professionals to coordinate patient care.
• Communicate regularly with treating clinicians regarding patient concerns, progress, and clinical updates.
• Provide patient and family support related to medical, social, emotional, insurance, and vocational needs.
• Document assessments, care plans, interventions, and updates accurately in the electronic medical record.
• Apply case management principles to promote cost-effective, high-quality, and continuous care.
• Consult with the manager regarding clinical, legal, ethical, and risk-management issues.
• Participate in quality assurance and performance-improvement activities in accordance with HUHS standards.
