Job summary

Harvard University Health Services (HUHS) seeks a licensed clinical social worker (LCSW) to coordinate medical, social, and vocational services for HUHS patients. The role emphasizes psychosocial assessments, outpatient care planning, and collaboration with clinicians to promote safe, timely care.

The position requires MA licensure, a master's degree, at least five years of experience, and strong communication, care coordination, and documentation skills.

Qualifications

• Master’s degree (required).
• Minimum of 5 years’ relevant work experience.
• Knowledge of discharge planning and care coordination.
• Strong organizational, analytical, problem solving and interpersonal skills.
• Experience in a hospital or healthcare setting preferred.

Responsibilities

• Collaborate with physicians, nurses, support staff and other healthcare professionals to coordinate patient care.
• Communicate regularly with treating clinicians regarding patient concerns, progress, and clinical updates.
• Provide patient and family support related to medical, social, emotional, insurance, and vocational needs.
• Document assessments, care plans, interventions, and updates in the electronic medical record.
• Apply case management principles to promote cost-effective, high-quality, continuous care.
• Consult with the manager regarding clinical, legal, ethical, and risk-management issues.
• Participate in quality assurance and performance-improvement activities.
