Job summary

ACL Digital is seeking a skilled ambulatory case manager in Cambridge, MA. The role delivers case management to HUHS patients, coordinating research, interviews, and care plans with clinicians and hospital teams.

You will educate staff on criteria/guidelines, monitor care utilization, and help identify quality issues within a small team that handles 40–50 cases and advance directives.

Qualifications

• Master's degree in Social Work is required.
• Current license to practice in Massachusetts (LCSW or LICSW).
• Case management certification preferred.

Responsibilities

• Provides ambulatory case management services to identified HUHS patients through a process of research, patient interview, monitoring of care utilization patterns and development of care plans.
• Collaborates with clinical staff to provide a comprehensive outpatient care plan.
• Communicates with treating clinicians on areas of concerns and provides clinical updates to clinicians on a regular basis.
• Participates in assuring that UHS is in the forefront of applying Case Management principles.
• Educates medical staff to criteria/guidelines as appropriate.
• Works with team leader to identify areas of concerns in clinicians practice patterns and assists in development of resolutions.
• Identifies potential quality of care issues. Assists in development of reports, resources required by other departments.
• Excellent organizational, analytical, problem-solving, and interpersonal skills.
• This is a small team that manages approximately 40-50 cases each, hospitalizations, and completes advance directive outreach.

Worker must provide a Letter of Good Health, signed by a physician, and an immunization record that shows compliance with HUHS Immunization Policy, to HUHS HR, on first day of assignment.
