You will document in the EMR, ensure culturally humble, patient-centered care, and participate in quality improvement. The ideal candidate holds a Masters in Social Work and MA LICSW/LCSW with strong organizational and communication skills.

Qualifications

• Masters degree and at least 5 years of relevant experience.
• Current Massachusetts LICSW or LCSW license.
• Intermediate Excel and Microsoft Office skills.
• Strong organizational, analytical, problem-solving, communication, and clinical assessment skills.
• Preferred: hospital or healthcare experience and knowledge of discharge planning and care coordination.

Responsibilities

• Coordinate care with physicians, nurses, support staff, community agencies, and other providers, with regular updates to treating clinicians.
• Provide patient and family support across medical, social, emotional, insurance, and vocational needs.
• Document assessments, care plans, and interventions promptly in the electronic medical record.
• Apply case management principles to promote cost-effective, high-quality, continuous care.
• Consult the manager on clinical, legal, ethical, and risk issues.
• Join quality assurance and performance-improvement activities.
• Adapt communication and care to accessibility, language, and developmental needs, and practice culturally humble, patient-centered, confidential care.
