Job summary

UI Health, Mile Square Health Center in Chicago, IL, seeks a Medical Social Consultant to coordinate patient services across the care continuum. The role involves counseling, crisis intervention, and helping patients access community resources within a collaborative team.

Responsibilities include brief screenings, care management planning, referrals, and ensuring timely services across the care team. Illinois residency is required and the position offers full benefits.

Qualifications

• Master's degree from an accredited school or university in social or behavioral science or a related health specialty area.
• Current State of IL Licensure as a Licensed Clinical Social Worker (LCSW) and/or Licensed Clinical Professional Counselor (LCPC).
• Illinois Residency is required within 180 days of employment.
• Preferred Qualifications: a minimum of 2 years of recent experience providing therapy services.

Responsibilities

• Participates as an integrated component within the primary care team by being available for warm-handoff brief screening and interventions.
• Actively assesses daily schedules of primary care clinicians to plan warm-handoff and brief interventions and co-locate with care teams.
• Conducts brief screening and targeted counseling and bridges patients to specialty mental health services when appropriate.
• Manages behavioral health referrals and message pool under the direction of the Main Behavioral Health Coordinator.
• Identifies service delivery problems and works with the interdisciplinary team to facilitate timely, cost-effective patient services.
