Role Overview

The purpose of a Medical Scribe at Oak Street Health is to support primary care providers with clinical documentation, allowing them to focus on delivering exceptional patient care. Scribes assist throughout the patient care journey, including planning, observing, documenting, and supporting post-visit activities.

These team members are also known as CISs (Clinic Informatics Specialists) and play a crucial role in accurate, timely clinical documentation. They become experts in our value-based care model, chronic condition management, ICD-10, and CPT coding, helping to identify and close care gaps. Extensive training is provided in clinical workflows, preventative care, documentation, and population health data streams.

Ideal candidates should commit at least 1-2 years to this role. It is a valuable opportunity for pre-med students, those interested in health informatics, public health, healthcare administration, medical coding, and related fields.

Responsibilities

1. Document Patient Encounters (~80%)
   • Join providers in exam rooms to observe and document visits.
   • Record comprehensive notes including history, assessment, plan, and physical exam.
   • Assign CPT and ICD-10 codes.
   • Prepare After Visit Summaries.
   • Consult with providers to ensure documentation accuracy.
2. Clinical Documentation Improvement (~10%)
   • Review medical records and support documentation enhancement.
   • Use population health tools to support care in a value-based model.
   • Assist in daily huddles and documentation reviews.
   • Collaborate with providers on documentation opportunities.
3. Administrative Support (~10%)
   • Place orders and referrals.
   • Address administrative tasks.
   • Support the care team with documentation-related responsibilities.
4. Perform other appropriate duties as assigned.

Qualifications

• Knowledge of medical terminology and medications (preferred from pre-med experience or clinical exposure).
• Prior clinical or shadowing experience (preferred).
• Prior scribe or transcription experience (preferred but not required).
• Strong listening and communication skills.
• Computer literacy and ability to learn workflows.
• Fluency in Spanish, Polish, Russian, or other community languages (where indicated).
• Ability to adapt to new workflows and learn quickly.
• Typing speed of 70+ words per minute (preferred).
• Willingness to work as part of a team and follow provider’s working style.
• Self-starter with excellent attendance.
• Commitment to at least 1 year (preferably 2+).
• Availability to work 40-45 hours per week during clinic hours.
• Compliance with policies including HIPAA.
• US work authorization.
