The Center for Inherited Blood Disorders (CIBD) is seeking a motivated Scribe to support its clinical team. This role involves accurately recording medical records, assisting with Telehealth, and revising documentation tools, contributing to a collaborative health environment. Job Summary
Reporting to the Clinic Supervisor, the Scribe will work directly with the clinical team to ensure accurate and complete medical records for each patient by recording patient interactions with the physician and other staff. Additionally, this position will work closely with the physicians and Clinic Manager to revise and update patient documentation tools in order to meet the needs of the clinic.
Department: Clinic Shift: Day Schedule: Monday - Friday (typically 9:00 - 5:30) Location: On-site
Essential Job Duties
Accompany physician/staff member into the patient examination room in order to transcribe a history and physical exam, and document accurately the physician’s/staff member’s encounter with the patient and others present listing all proper diagnoses and symptoms; laboratory tests and radiology tests; and, medications as well as follow up instructions and prescriptions as dictated by the physician/staff member. Revise & complete documentation within a timely manner.
Update and revise forms and documents utilized by clinic staff members as directed by the physicians and Clinic Manager. Track ongoing documentation to remain organized and timely with completion.
Transcribe and document for meetings as requested (Comprehensive weekly meetings, clinical staff meetings, etc.).
Coordinate execution of Telehealth visits ensuring all involved staff, physician and patient are logged in and ready at the scheduled time. Transcribe for Telehealth and non-clinical patient visits/meetings (such as patient interventions).
Prepare charts and documentation prior to the start of each clinic. Summarize the day’s visits and disseminate a daily summary to all staff involved in patient care.
Required Education/Experience
High school diploma or equivalent. (Required)
Bachelor’s degree or equivalent education (Preferred)
One of the following:
6 months experience as a Medical Scribe.
6 months experience as a Medical Assistant
6 months related experience and/or training, or equivalent combination of education and experience.
Skills
Medical terminology.
Knowledge of general exam note structure (Interval History, Physical Exam, etc.).
Recognition of physical exam process and ability to record exam details.
Excellent English language documenting and writing skills.
Computer proficiency and ability to quickly learn new applications.
Organizational skills with focus on tracking patient care and improving patient flow.
Professional demeanor and recognition of privacy considerations for patients and families.
Licensure/Certification/Registration:
Basic Life Support
HIPAA training
Current CA Medical Assistant Certificate (preferred)
Compensation offered will be determined by factors such as location, level, job-related knowledge, skills, and experience.
About the Organization The Center for Comprehensive Care & Diagnosis of Inherited Blood Disorders (CIBD), a 501(c)3 non-profit organization, is committed to providing comprehensive care for patients and their families with rare, chronic inherited blood disorders. CIBD is a part of a nationwide network of over 140 Hemophilia Treatment Centers that use multidisciplinary teams of blood disorder specialists to provide expert comprehensive diagnosis, medical care, outreach, education, outcomes monitoring, surveillance, and prevention services to people with hemophilia, Sickle-Cell Anemia, Thalassemia, Von Willebrand Disease, and other related hematology conditions. CIBD is the regional grantee for the Western States Region/Region IX providing regional leadership, fiscal and administrative oversight, grants management, capacity building, clinical education, technical assistance, evaluation, and outreach to public and private health agencies at the local, state, and federal levels.
