Job Summary
This person will ensure proper documentation of clinical services for providers. The scribe will be present during the provider's performance of clinical service and document on behalf of the physician the clinically relevant findings as directed by the provider during the course of the service. This person will be filling out paperwork, summaries and referral letters after a visit and entering data into medical records.The scribe position is not one that can examine the patient in any clinical capacity, nor interject his/her own direct patient care observations or impressions in the scribed documentation. Other duties include:
- Scribe in real-time in the exam room to provide enhanced interaction between patient and provider.
- Familiarity with medical terminology to translate jargon and abbreviations to become part of a patient's electronic medical file.
- Responsible for documenting all information obtained from the patient, services/testing done, and results of any services/testing in the Electronic Medical Record(EMR).
- Adhere to legal requirements for medical report style and confidentiality requirements.
- Comply with all national and local PHI laws including HIPAA and HITECH.
- Comply with all facility, department, and company policies and procedures.
- Consulate with the care team and other providers on patient needs.
- Scribes do not participate in any patient care and should refer all requests related to patient care to the responsible provider or nursing staff.
- Perform other duties as assigned.

Qualifications
- A High School Degree or GED equivalence.
- Certified medical transcriptionist (CMT) status is preferred, but not required.
- Clinical certificates, such as MA or EMT, are preferred but not required.
- Experience working in a healthcare setting, preferably in outpatient health services.
- Computer experience including familiarity with EMR software.
- Excellent written and oral communication skills, including English usage, and grammar.
- Familiar with medical terminology, anatomy, and physiology, clinical medicine.
- Ability to operate designated word processing, dictation, and transcription equipment.
- Fully vaccinated against the Covid-19 virus in accordance with Federal CDC and State/Local regulatory guidance.
