Job Description: Medical Scribe at Oak Street Health

The purpose of a Medical Scribe at Oak Street Health is to support our primary care providers with clinical documentation, enabling them to focus on delivering exceptional patient care. Scribes assist providers throughout the patient care journey — from morning huddles to plan the day's visits, joining in exam rooms to observe and document, and following up after visits to support next steps.

Beyond traditional scribing duties, these team members serve as clinical documentation assistants, known internally as CISs (Clinic Informatics Specialists). They play a crucial role in ensuring accurate, specific, and timely clinical documentation. Scribes observe and document patient encounters in real time, become experts in our value-based care model, and support the documentation and management of chronic conditions, including ICD-10 and CPT coding. They leverage this expertise to help providers identify and close care gaps. Extensive on-the-job training is provided in clinical workflows, value-based medicine, preventive care, population health data, and team-based care.

Given the reliance of patients and providers on Scribes, ideal candidates should commit at least 1-2 years to this role. This position offers valuable, paid clinical experience for pre-med students and those pursuing careers in Health Informatics, Public Health, Healthcare Administration, Medical Coding, and related fields.

Responsibilities

1. Join providers in exam rooms to observe patient visits.
2. Document patient encounters, including history, assessment, plan, and physical exam, in structured notes.
3. Assign appropriate CPT and ICD-10 codes.
4. Prepare After Visit Summaries.
5. Collaborate with providers to ensure accurate and detailed documentation.
6. Support clinical documentation improvement (~10%), including requesting and reviewing medical records, leveraging population health tools, and participating in daily huddles and reviews.
7. Provide administrative support to providers and care teams (~10%), such as placing orders and referrals.
8. Address additional tasks related to clinical documentation as needed.

Qualifications

Knowledge

• Understanding of medical terminology and common medications (from a pre-med program or clinical experience) — required.
• Prior clinical experience, including shadowing or volunteering — strongly preferred.
• Scribe or transcription experience — preferred but not required.

Skills

• Excellent listening and communication skills — required.
• Strong computer literacy and adaptability to new workflows — required.
• Fluency in Spanish, Polish, Russian, or other languages spoken by our communities — required where applicable.

Abilities

• Ability to adapt quickly to new workflows and learn new skills — required.
• Typing speed of 70+ words per minute — strongly preferred.
• Willingness to take direction and work as part of a team providing patient care — required.
• Self-starter with a proactive approach — required.
• Excellent attendance and in-person availability at clinics — required.
• Commitment to at least 1 year in the role (2+ preferred) — required.
• Availability to work approximately 40-45 hours per week during clinic hours with predictable schedules — required.
• Adherence to hospital and Oak Street policies, including HIPAA — required.
