Job summary

Mirage Post Acute is seeking a Med record assistant with an LVN background to maintain resident medical records and health information systems in compliance with federal and state guidelines.

You will support the medical records department, index and file records, and assist with scheduling assessments and ensuring chart accuracy. Strong organization and confidentiality are essential, with on-site work and general duties as assigned.

Qualifications

• Must have a high school diploma or GED.
• Experience with medical records systems preferred.
• Strong organizational skills and ability to handle confidential information.

Responsibilities

• Receive and follow work schedules and policies.
• Assist in organizing the medical records department and maintain records.
• Retrieve, file, and update resident charts and documents.
• Index records and ensure proper filing and privacy compliance.
• Assist in scheduling assessments and coordinate with MDS team.
• Record incidents and maintain records according to policy.
• Ensure records are complete, signed, and properly stored.
• Handle chart/record requests from authorized personnel.
