Job summary

Hospice Home Care seeks a meticulous Medical Records Clerk to support our nursing and care teams. You will enter and maintain patient data, verify documents, and resolve discrepancies to ensure timely delivery of edited records.

Working closely with facility staff and the pharmacy consultant, you will safeguard confidentiality and comply with corporate policies while contributing to high-quality hospice care.

Responsibilities

• Ensures timely entry of medical data into computer systems and editing documentation for client facilities.
• Maintains accurate updates and medical records documents for each facility.
• Researches unedited sheets to resolve illegibility or unfamiliar terminology used by facilities.
• Recognizes deviations and irregularities in data and seeks resolution from originator or manager.
• Verifies and clarifies problems or inquiries with facility documentation requests/needs.
• Communicates with nursing staff, Directors of Nursing and Administrators regarding corrections of medical data.
• Uses reference materials to improve skills regularly.
• Maintains confidentiality of patient demographics and medical information.
• Runs medical records forms and reports containing facility, patient and pharmaceutical information.
• Assists consultant pharmacists by providing facility data and informing on department procedures.
• Promotes customer goodwill and enhances corporate image to support the mission and values.
• Conducts audits with facilities on request using computerized data.
• Observe and comply with PharMerica policies and procedures.
