Job summary

Affinity Hospice Management LLC is seeking a Medical Records Clerk to support the clinical department with recordkeeping, chart audits, and reporting. You will enter admits, changes in care level, discharges, and physician orders into the EMR, scan documents, and ensure timely updates for IDG meetings.

The role requires accurate data entry, strong organization, and the ability to work within a healthcare team to maintain complete patient charts and prepare records for storage.

Qualifications

• High school diploma or equivalent.
• 1 year medical records experience (home health preferred).
• Strong interpersonal and organizational skills.
• Flexible, cooperative and self-directed in fulfilling roles.
• Ability to function as a member of an integrated healthcare team.

Responsibilities

• Maintains and updates patient files in EMR.
• Completes chart admission audits and communicates issues to Clinical Director.
• Inputs updates on clinical status, insurance, level of care, staff assignments, or discharge.
• Scans and files clinical data, signed orders, and consents.
• Ensures care plans, assessments, and recertifications are updated for IDG meetings.
• Tracks certifications and physician orders; runs reports for unsigned orders.
• Maintains patient on-call book and contacts physician offices for information.
• Assists with annual reports and other patient data requests.
• Receives and compares visit logs to contact visit reports weekly.
• May assist with tracking DME and medical supplies.
• Participates in QA and PI activities.
