Job summary

Ohio Living is seeking a Medical Records Associate to maintain a complete medical records program in accordance with company standards, federal, state, and local guidelines and regulations.

You will review records on nursing units, manage the EMR, and assist in insurance appeals and re-certifications while ensuring timely, accurate documentation and confidentiality across the organization. This role requires accuracy, English proficiency, and adherence to policy.

Qualifications

• High school diploma or equivalent required.
• Associate degree in health information or related field preferred.
• Two years experience in medical records in a long-term care, hospital, or other related healthcare setting required.
• Must have working knowledge of diagnosis codes.

Responsibilities

• Reviews all records on the nursing units and provides clerical support for the nursing management team.
• Assists in planning, developing, organizing, implementing, evaluating, and directing the medical records per policies and procedures; collects, scans, and attaches information to the EMR.
• Audits EMR for completeness and reports findings to DON/Admin/Unit Manager; gathers documentation for surveys and insurance re-certifications as assigned.
• May initiate, prepare, and ensure completion of Medicare certifications and re-certifications; notifies residents, patients, and families of non-coverage when applicable.
• Keeps confidentiality of resident and patient information and supports compliance with federal, state, and local regulations.

Education
