Canyon Springs Post Acute in San Jose, CA is seeking an experienced Medical Records Director to oversee the records department, ensure HIPAA compliance, and maintain accurate resident documentation. The role requires strong leadership and knowledge of health information management in a skilled nursing/long-term care setting.

You will supervise staff, coordinate with clinical and administrative teams, and manage EHR systems while maintaining confidentiality and regulatory readiness.

Qualifications

• Experience in medical records management with HIPAA and regulatory knowledge.
• Ability to supervise staff and maintain high documentation standards.
• Familiarity with electronic health record systems and audits.
• Strong organizational and communication skills for cross-department collaboration.

Responsibilities

• Oversee creation, organization, storage, and retrieval of resident records per regulations.
• Ensure timely documentation of admissions, discharges, and clinical updates.
• Monitor records for completeness, accuracy, and policy compliance.
• Coordinate with nursing, rehab, and admin teams to ensure on-time completion.
• Manage records requests from residents, families, providers, and agencies.
• Oversee EHR systems and resolve documentation issues as needed.
• Conduct audits and prepare reports for audits and inspections.
• Lead development of records policies and procedures and train staff.
