h3>Job Summary
The Medical Records Clerk organizes and evaluates patients' health information and records. They perform various record-keeping and clerical duties to support services provided in a medical setting.

Essential Job Functions

  1. Sort and save electronic faxes in patients' charts daily and timely.
  2. Manage and prepare the Fax Server; index patient charts; gather and document information from patients or providers, ensuring accurate documentation in patient charts (e.g., specialists).
  3. Ensure medical records are organized, accurate, and complete, following proper naming conventions per policy.
  4. Create digital copies of paperwork and store records electronically.
  5. Safeguard patient records and ensure compliance with HIPAA standards.
  6. Transfer data into the clinic's main system database.
  7. Retrieve and route records to appropriate personnel or departments.
  8. Prepare reports and forms according to established policies.
  9. Maintain knowledge of chart control, access, and storage policies and regulations.
  10. Process requests for medical record releases and maintain logs per office policy.
  11. Perform administrative duties such as answering phones, faxing, filing confidential documents, and basic internet/email use.
  12. Provide excellent customer service to internal and external customers.
  13. Perform other duties as assigned.
Minimum Education, Experience & Skills
  • High school diploma or GED.
  • Strong data entry and computer skills.
  • Attention to detail and customer service focus.
  • Effective communication skills.
  • Teamwork orientation and organizational skills.
  • Knowledge of medical terminology, ICD-10, and CPT coding.
  • Proficiency in electronic health records.
  • Ability to remain calm in emergencies.
  • Basic math skills and safe work practices.
Preferred Education, Experience & Skills
  • 2-3 years in a medical customer service role.
  • Bilingual in English/Spanish preferred.
Physical & Mental Requirements
  • Ability to lift up to 50 pounds and push/pull up to 100 pounds.
  • Ability to stand or sit for extended periods.
  • Use of fine motor skills for operating equipment.
  • Ability to drive and operate a vehicle.
  • Ability to receive and comprehend instructions.
  • Logical reasoning for problem solving.
  • Exposure to communicable diseases or bodily fluids occasionally.
  • Occasional travel may be required.
