Description

The Medical Records Coordinator is responsible for ensuring that all medical orders, assessments, and reassessments are tracked and completed in a timely manner and properly filed. Specific duties include:

  1. Verify the client provider name with the Registered Nurse and/or the patient.
  2. Verify provider license details, including name, profession, license number, NPI number, and practicing address.
  3. Check for expired or revoked providers’ licenses regularly via the New York Office of Professions.
  4. Add physician information into the home care data system.
  5. Maintain a master tracking list for assessments and MD orders (485).
  6. Ensure the accuracy of all information on the patient’s 485 or interim orders by confirming the registered nurse visit date and provider name before sending for review and signature.
  7. Track MD orders to ensure they are received back signed by the physicians.
  8. Communicate with DPS regarding any pending MD order signatures.
  9. Maintain communication with relevant parties to expedite MD order processing.
  10. Coordinate with the Director of Patient Services and nursing supervisor regarding outstanding orders and assessments.
  11. Assist with clinical projects, including audits.
Competencies
  • 2-5 years of experience in homecare or related field.
  • Bilingual in English and Spanish preferred.
  • Knowledge of Human Resources Administration (HRA) Web applications and home care data systems like SANDATA or HHAeXchange.
  • Problem-solving skills and ability to multitask.
  • Excellent verbal and written communication skills.
  • Strong organizational and customer service skills.
  • Knowledge of home care practices within an LHCSA.
  • Ability to maintain confidentiality.

  • Proficiency in Microsoft Office, especially Excel, and experience with web-based applications.
Education & Experience

High School Diploma or equivalent required; Bachelor’s degree preferred.
