Job summary

Telos Health Systems is seeking a Medical Director & Skilled Nursing Facility Primary Care Physician (MD/DO) to provide on-site care in Memphis, TN, and to lead clinical teams across multiple facilities. The role includes CMD training, leadership responsibilities, and collaboration with NPs/PAs to maintain high-quality post-acute care.

Qualifications

• MD/DO required.
• Active DEA license.
• Active Tennessee medical license or ability to obtain.
• Interest in post-acute care, leadership, and community-based medicine.

Responsibilities

• Provide on-site primary care to skilled nursing facility residents.
• Serve as Medical Director for assigned facilities and participate in CMD training.
• Collaborate with interdisciplinary care teams and facility leadership; supervise NP/PA clinicians.
• Support QA & PI initiatives and regulatory compliance; participate in after-hours telehealth care.

Education

Doctor of Medicine (MD) or Doctor of Osteopathy (DO)

Board certified in Internal Medicine

Board certified in Family Medicine

Board certified in Geriatrics

Board certified in Emergency Medicine
