Medical Director – Physician

The Personalized Health Partners medical practice is currently recruiting a physician to serve as Medical Director at our Cincinnati, OH facilities. This position covers Chamberlin Healthcare Center, Parkview Healthcare Center, Kenwood Healthcare Center, and Forest Hills Healthcare Center.

As Medical Director, you will be part of an innovative care model that combines traditional bedside manner with advanced technologies to improve patient outcomes and reduce hospital readmissions.

Job Duties And Responsibilities

• Implement medical services, policies, and procedures in compliance with regulations; oversee post-acute and long-term care practices.
• Coordinate medical care, emergency procedures, liaise with physicians, and monitor quality of care.
• Provide physician services within skilled nursing facilities and clinics.

Qualifications

• M.D. or D.O. required
• Gerontology certification preferred
• Ohio medical license
• Board Certified in Internal Medicine, Family Practice, Geriatrics, or Hospice/Palliative
• 5+ years experience with geriatric or long-term care populations
• Experience working with and educating Nurse Practitioners

Preferred Qualifications

• Healthcare management/leadership experience
• Knowledge of CMS, Medicare Advantage, SNP regulations
• Experience with HCC, Risk Adjustment, utilization metrics, and bed management
• State-specific practice agreements and licensing
• Familiarity with EMR systems
