The Medical Director – Physician at Personalized Health Partners is currently recruiting a physician to serve as Medical Director at our Cincinnati, OH facilities, including Chamberlin Healthcare Center, Parkview Healthcare Center, Kenwood Healthcare Center, and Forest Hills Healthcare Center.

As Medical Director, you will be part of an innovative care model that combines traditional bedside manner with advanced technologies to reduce hospital readmissions and improve patient outcomes.

Job Duties and Responsibilities:

1. Implement medical services, policies, and procedures in compliance with regulations; oversee post-acute and long-term care medicine.
2. Coordinate medical care, including emergency procedures, liaise with attending physicians, and maintain quality surveillance.
3. Provide physician services within skilled nursing facilities and clinics.

Qualifications:

• M.D. or D.O. required.
• Gerontology certification preferred.
• License in Ohio.
• ABMS Board Certification in Internal Medicine, Family Practice, Geriatrics, or Hospice/Palliative.
• 5+ years experience with geriatric or long-term care populations.
• Experience working with and educating nurse practitioners.

Preferred Qualifications:

• Healthcare management/leadership experience.
• Familiarity with CMS, Medicare Advantage, SNP regulations.
• Knowledge of HCC, risk-adjusted reimbursement, utilization metrics, and licensing requirements.
• Knowledge of EMR systems.
