Qualifications

• Physician with MD or DO and board certification in Hospice and Palliative Medicine.
• Experience leading or supervising clinical teams in palliative care.
• Comfort with value-based, home-based care models and data-driven improvements.

Responsabilités

• Lead the local care team in California, supervise clinicians, and serve as collaborating physician of record.
• Spend significant time in direct patient care via virtual and in-person visits, focusing on symptom management and goals-of-care conversations.
• Design and refine Daymark's supportive care model and establish referral criteria and quality measures.
• Build partnerships with oncology practices, primary care, hospice, home health, and health plan partners.
