Job summary

Impact Health PC is seeking a Medical Director and/or Attending Physician to provide high-quality medical care in Skilled Nursing Facilities around Litchfield, CT. This role offers leadership opportunities, clinical autonomy, and strong earning potential.

Ideal candidates hold an MD/DO with CT licensure, board eligibility in Internal Medicine, Family Medicine, or Geriatrics, and experience in long-term care or geriatrics.

Qualifications

• MD or DO from an accredited medical school.
• Active, unrestricted Connecticut medical license.
• Board Certified or Board Eligible, preferably in Internal Medicine, Family Medicine, or Geriatrics.
• Current state and federal prescribing registrations, including controlled substance authority as required.
• Eligible to participate in Medicare and Medicaid programs.
• Experience in Skilled Nursing, Long-Term Care, Geriatrics, or Hospital Medicine preferred.
• Strong clinical leadership, communication, and organizational skills.
• Knowledge of long-term care regulations and quality standards preferred.

Responsibilities

• Provide comprehensive medical care to residents in Skilled Nursing Facilities.
• Serve as Attending Physician and/or Medical Director based on facility needs.
• Provide clinical leadership, medical oversight, and guidance to facility staff and providers.
• Collaborate with nursing staff, advanced practice providers, and interdisciplinary care teams.
• Review patient care plans, clinical outcomes, and quality improvement initiatives.
• Participate in QAPI programs and ensure regulatory compliance.
