WelbeHealth is seeking a Medical Director II to lead medical care for PACE participants, overseeing the IDT and ensuring QI and policy compliance. The role emphasizes value-based care and development of innovative care pathways to improve participant outcomes and integration across services.
The position includes executive leadership responsibilities, supervision of physicians and clinicians, and a rotating on-call schedule with home visits. Competitive compensation and benefits are provided.

We employ a collaborative interdisciplinary team (IDT) approach to evaluate and guide participant care.
