Job summary

WelbeHealth, LLC. is seeking Primary Care Physicians for a value-based PACE program. You will perform comprehensive in-person assessments with the interdisciplinary team to develop personalized care plans for Medicare and Medi-Cal participants.

Qualifications

• Must complete residency program accredited by the ACGME.
• MD or DO degree required; Board certification preferred.
• Active state physician license without restrictions required.
• State CDS certification and DEA registration.
• CPR and First Aid certification within 180 days of hire preferred.

• Perform comprehensive in-person assessments with IDT to develop personalized care plans.
• Coordinate discharge for acute, short-term, and long-term placement.
• Focus on comprehensive care with about 8 patient visits per day; no hospital rounds.

Essential job duties include:

• Perform comprehensive in-person assessments, leading and partnering with the interdisciplinary team (IDT) to develop a personalized care plan for each participant
• Coordinate discharge for acute, short-term, and long-term placement
• No hospital rounds – focus on comprehensive care, seeing an average of 8 patients a day

Job requirements include:

• Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
• Master's or doctorate Degree
• Board Certification preferred
• Active state physician license without restrictions
• State CDS certification and DEA registration
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred

We are seeking Primary Care Physicians in Internal Medicine, Family Medicine, Emergency Medicine or Geriatrics. Must be eligible for enrollment to provide services to Medicare and Medi-Cal participants. Board certification preferred.

Compensation consists of base salary plus bonus. WelbeHealth offers a competitive total rewards package that includes a 401(k) match, comprehensive healthcare coverage, and a broad range of additional benefits. Actual compensation will be determined based on experience and relevant qualifications.

$280,737.60 - $330,595.20 USD
