Job summary

Eventus WholeHealth seeks an Attending Physician to lead population health within our Outcomes-Centered Healthcare model across skilled nursing and assisted living facilities.

You will collaborate with Advanced Practice Clinicians and facility leadership to deliver high-quality, team-based care and may take Medical Directorship responsibilities to provide clinical leadership and oversight.

Qualifications

• MD or DO required; board certification in Internal Medicine, Family Medicine, or Geriatrics preferred.
• CMD certification a plus; required within 2 years in Medical Directorship.
• Active and unrestricted medical license; DEA registration as required.

Responsibilities

• Lead population health stewardship in the Outcomes-Centered Healthcare model across assigned facilities.
• Collaborate with Advanced Practice Clinicians and facility leadership to ensure high-quality, team-based care.
• Potentially serve in Medical Directorship providing clinical leadership and oversight.
