The Integrated Care Center Clinic is a federally qualified health center that provides primary care, behavioral health, addiction treatment, psychiatry, dental care, and more. We care for those most in need, including people suffering from homelessness, mental illness, substance use, and chronic medical conditions. We provide team‑based, integrated care. Key Responsibilities
Leadership Responsibilities
  • Develop, implement, and oversee clinic site budgets in conjunction with HR360 executive leadership
  • Participate in regular meetings with executive leadership to review budgets, utilization, and quality targets and progress toward goals
  • Develop and implement policies pertaining to the Medical Department
  • Provide medical supervision and oversight of quality of care among providers and ancillary staff in clinical areas
  • Promote and enhance integrated team‑based care, including patient‑centered care
Medical Direction Responsibilities
  • Develop and implement policies pertaining to the Medical Department
  • Provide medical supervision and oversight of quality of care among providers and ancillary staff in clinical areas

  • Ensure that in the absence of a primary care provider, a mechanism is established so a physician is available to review lab and diagnostic reports, prescriptions, and follow up on medical care
  • Supervise and promote continuous quality improvement efforts
Clinical Staff Direction Responsibilities
  • Provide supervision of medical providers
  • Strong primary care foundation and support clinical training of providers
  • Review employee performance, provide feedback, and make recommendations for individual performance improvement and development for medical providers and other staff during regular supervision and annual performance appraisal
  • Orient new providers, monitor performance during initial employment and thereafter through chart audits, utilization review and other processes
  • Perform annual performance appraisal of all direct report providers
  • Provide supervision to clinical leadership (e.g., RN manager, MA manager)
Agency Development/Support Responsibilities
  • Collaborate with HR360 executive leadership to enhance revenue portfolio and advance business development for the medical clinics
  • Provide guidance for credentialing and privileging of clinical staff
  • Attend and participate in behavioral health case management, substance use disorder management, and clinical collaboration activities
  • Participate in clinic leadership team activities
  • Precept medical students, NP/PA students, residents, and fellows; educate, train, and supervise volunteers as necessary to maintain and build volunteer programs
Education and Knowledge, Skills and Abilities
Education and Experience Required
  • M.D. or D.O. degree from an accredited U.S. medical school
  • Successful completion of internship and residency from an accredited program
  • 1–3 years experience as a primary care provider
  • Leadership experience required
Certification/Licensure
  • Board certified in family medicine or internal medicine
  • Valid and active California medical license, with no limitations on the license in the designated specialty
  • Eligible to participate in CMS Medicaid/Medicare reimbursement programs
  • Valid and active DEA license with X waiver to prescribe buprenorphine; if not currently holding an X waiver, must obtain one within six months of employment
Desired Skills and Experience
  • Experience working with patients with addiction
  • Experience working with patients that have a history with the criminal justice system
  • Experience integrating safe management of pain syndromes within primary care
  • Knowledge of third‑party payer requirements, California practice guidelines, malpractice laws, Title 22, Bureau of Primary Care, and state funding sources (preferred)
  • Experience working in FQHC and familiar with FQHC compliance requirements
  • Bilingual in Spanish
