Trinity Health PACE in Baton Rouge/Lafayette, LA seeks a Medical Director to lead clinical services for older adults in a on-site setting. You will oversee outcomes, guide providers, and drive performance improvements across the program, ensuring high-quality, compassionate care.

The role requires MD/DO with Louisiana license, board certification in a primary care field, and leadership experience, with an emphasis on geriatrics. On-call duties are part of the rotation.

Qualifications

• MD or DO with active state license in Louisiana.
• Board certification in Internal Medicine or Family Medicine (Geriatrics preferred).
• 1+ year experience with frail or elderly populations.
• Prior leadership or supervisory experience.

Responsibilities

• Provide medical leadership and supervision for participant care.

• Lead and support the QAPI program.
• Collaborate with hospitals, SNFs, and community providers.
• Mentor and coach clinical staff.

Position Purpose

Join Trinity Health PACE as a Medical Director and lead a team dedicated to delivering compassionate, high-quality care to older adults. You’ll oversee clinical outcomes, guide Primary Care Providers, and drive performance improvement across our program.

What You Will Do

Minimum Qualifications

• MD or DO with active state license in Louisiana

• Prior leadership or supervisory experience
• Valid driver’s license, insurance, and reliable transportation
• Current BLS, CDS Certification, and DEA Registration
