Job summary

Trinity Health PACE seeks a Medical Director to lead a provider team in Baton Rouge/Lafayette, LA, delivering compassionate, high-quality care to older adults. You will oversee clinical outcomes, guide Primary Care Providers, and drive performance improvement across our program.

Responsibilities include medical leadership, quality oversight, mentoring staff, and collaboration with hospitals and community partners. Ensure documentation, EMR optimization, and active on-call participation.

Qualifications

• MD or DO with active state license in Louisiana.
• Board certification in Internal Medicine or Family Medicine; geriatrics preferred.
• Experience leading care teams and supervising clinical staff.
• Current BLS, CDS Certification, and DEA Registration.

• Lead and support the Quality Assurance Performance Improvement (QAPI) program.
• Collaborate with hospitals, SNFs, and community providers.

• Ensure proper documentation and EMR optimization.
• Support pharmacy coordination and advance care planning.

Position Purpose

What You Will Do
