Job description

The Staff Physician provides direct patient care in an effort to ensure that patient health care needs are met at all times.

Responsibilities
  • Provides direct patient care, develops treatment plans, and assesses patient's response to treatment while ensuring the highest quality of healthcare services.
  • Participates in coordination of a 24‑hour care delivery system, including night and weekend on‑call responsibilities.
  • Clinical Care for Participants
    • Performs and documents comprehensive history and physicals on new referrals.
    • Performs and documents interval history and physicals on established participants; completed every 6 months or as indicated.
    • Participates in the development and revision of participants' Plan of Care with the interdisciplinary team and integrates the primary care treatment plan into the overall plan of care.
    • Discusses and coordinates ongoing care with PACE participants and their family/caregivers, including development of an Advance Care Plan.
  • Collaboration and Partnership
    • Coordinates admission of participants to hospital with the hospital medicine service; provides consultative management of hospitalized patients; provides updates to the team; assists with discharge planning.
    • Collaborates with other physicians to ensure high‑quality medical care; determines need and facilitates referrals of participants to medical specialists.
    • Collaborates with and supervises midlevel practitioners to ensure comprehensive and continuous care.
    • Manages all care of participants in the nursing home in coordination with the midlevel practitioner; makes regular visits; performs telephone contacts; coordinates nursing home participants’ admission to hospital when necessary.
    • Acts in family conferences, team meetings, committees, and work groups; communicates participant changes.
    • Participates in educational programs, in‑service meetings, and training sessions to share expertise.
  • Quality and Performance Improvement
    • Investigates new techniques, practices, and equipment to maximize quality and efficiency of care.
    • Assists in the development and implementation of continuous quality improvement programs, ensuring services are performed with compassion, understanding, and respect.
    • Completes all documentation in the electronic medical record as required; complies with CMS regulations and guidelines.
    • Promotes quality and efficiency of own performance by staying current with latest trends through seminars, conferences, and professional affiliations.
    • Participates in PACE Baton Rouge Committees and advises the Medical Director on improving accountability of services.
  • Other Duties
    • Maintains flexibility in schedule and responds to unexpected emergencies and changes in workload.
    • Performs other duties as assigned or requested.
Qualifications
  • Experience: 1 year working with frail elderly.
  • Education: Graduate of an accredited School of Medicine; completed residency in Internal Medicine or Family Medicine.
  • Licensure: Current unrestricted Louisiana Medical License.
