Community Mainstreaming Associates is seeking a Medical Counselor to support residents with developmental disabilities in East Meadow and Massapequa, NY. The role focuses on medical oversight, medication administration, and ensuring health and safety while promoting independence.

You will document services in the electronic health record, collaborate with the RN and interdisciplinary team, attend IDT meetings, and participate in life plans.

Formación

• High school diploma or GED.
• Minimum of 2 years of experience working with individuals with I/DD.
• Current AMAP certification.
• Knowledge of OPWDD medical and medication regulations.
• Strong written and verbal communication skills.
• Familiarity with electronic health records and electronic documentation systems.
• Valid New York State Driver's License.
• Smartphone required.

Responsabilidades

• Assist residents with personal care, nutrition, hygiene, and meal planning.
• Promote resident health and safety while providing education regarding medical conditions and medications.
• Administer medications, including insulin and tube feeding as required by residents' plans, per NY AMAP regulations.
• Assist residents with scheduling medical appointments and advocate during medical visits.
• Ensure medical follow-ups are communicated to the RN and Residence Manager.
• Provide first aid and emergency medical treatment within scope of training.
• Record medical and medication updates, behavioral supports, and daily goal implementation.
• Attend monthly staff meetings and IDT meetings.
• Participate in residents' Life Plan meetings.
• Maintain confidentiality and ensure records are handled per regulations.
