Job summary

SCO Family of Services is seeking a Medical Coordinator/Direct Support Professional in Hollis, NY. This full-time, on-site role pays $21 USD per hour.

You will work closely with the RN and Residence Manager to schedule medical appointments, prepare forms, and ensure documentation is up to date. Responsibilities include coordinating weekly/monthly medical calendars, ensuring staff coverage for appointments, accompanying individuals to visits when possible, and maintaining accurate MAR and medical

Qualifications

• Schedule medical appointments in a timely manner per regulations.
• Collaborate with RN and Residence Manager to ensure staffing for appointments.
• Accompany individuals to medical appointments when possible.

Responsibilities

• In collaboration with the RN, ensure that all medical appointments are scheduled in a timely manner and in accordance with regulations.
• Prepare clinical consultation forms for all medical appointments.
• Ensure that there are sufficient staff scheduled for each appointment so the appointment can be attended in a timely manner.
• Accompany individuals and/or staff on medical appointments whenever possible.
• Ensure that all medical documentation is reviewed and signed by the RN and/or Residence Manager and filed in the individual’s medical binder.
